Case Study: Minimally Invasive Liver Biopsy in a Dog with Progressive Hepatopathy

An Advanced Surgery Case Study by Katherine Neal, DVM, MS, DACVS

 

The Case

A senior canine patient was referred to our surgery service for diagnostic evaluation after persistent elevation of liver enzymes despite appropriate medical management. Previous diagnostic workup, including abdominal ultrasound, bile acid testing, and coagulation screening, had not established a definitive diagnosis, and the patient remained clinically asymptomatic.

The goal of surgery was to obtain high-quality diagnostic tissue while minimizing patient morbidity and facilitating a rapid recovery.

 

Surgical Findings

Diagnostic laparoscopy revealed multiple red-to-tan nodules distributed throughout several hepatic lobes without evidence of other significant intra-abdominal abnormalities.

Using a Single Incision Laparoscopic Surgery (SILS) approach, Dr. Katherine Neal obtained multiple full-thickness liver biopsies from separate hepatic lobes. Samples were submitted for:

  • Histopathology
  • Quantitative hepatic copper analysis
  • Aerobic culture
  • Anaerobic culture

Following confirmation of hemostasis, the procedure was completed through the same small incision. Anesthesia and recovery were uneventful.

Outcome
